- New Patient Documents (if you’re unable to print them at home please arrive 10 minutes early to fill them out)
- Current prescriptions of glasses and sunglasses (if you have them)
- Current prescription of contacts and/or contact lens boxes
- List of prescriptions you take
- Sunglasses (post dilation)
- Copays are due at the time services are provided

For those insurance plans that we are not providers for, payment is due on the day of service. We will be happy to supply the necessary paperwork which the patient may then submit for reimbursements.
