1. New patient registration forms are available here. You may print them out and bring them into the office to save time registering.
   
   
2. Be sure to bring your insurance cards, including general health insurance and vision plan ID cards.
   
   
3. If you wear contact lenses, please wear your lenses into the exam. If you wear disposable contact lenses bring in your current contact lens prescription or the boxes in which your lenses came.
   
   
4. Please bring your eyeglasses, sunglasses and medication list.
   
   
5. Your eyes will likely be dilated, so bring sunglasses (if you have some) to wear when leaving the office. If you do not have sunglasses we have post dilation lenses you may wear.
   
   
6. Expect to spend about 1 hour at the office unless you are selecting eyewear or wear contact lenses, which means you may be here longer.
